Staff will help you make a medical appointment. You will be asked to provide your name, address, phone number, insurance information.

Arrive a few minutes early for your first appointment to fill out the short registration form and health history information. Please bring your Social Security card and private insurance/Medicaid/Medicare card(s).

If you don't have health insurance.
If you do not have health insurance, you may qualify for one of our discount programs that will allow you to access health care at discounted rates. You will be asked to meet with one of our intake personnel to determine if you and/or your family qualify for the programs based on family size and income. One of customer service can explain the details of applying for our programs. You must provide proof of income to apply for the programs.
