Watts Healthcare’s Healthy Aging and Wellness program is more than a medical program. It is a whole person approach to healthy aging. We believe true wellness includes physical, emotional, and social health, and we provide care that reflects that vision.

Through our Chronic Care Management (CCM) program, participants with two or more ongoing health conditions receive additional support from a dedicated Care Coordinator. Your Care Coordinator checks in regularly, helps track your health, answers questions, and coordinates care with your other providers to keep you supported between visits.

We also complement this care by creating safe, inclusive and welcoming spaces where our seniors can engage and feel valued, beyond the four walls of an exam room.

 

 

Chronic Care Management (CCM) is a Medicare service that helps you manage your health outside of regular appointments. If you are living with two or more chronic conditions, our care team offers ongoing guidance, check-ins, and coordination to help you stay healthy between visits.

As part of Chronic Care Management (CCM), you will have a dedicated Care Coordinator who connects with you monthly. These conversations give you a chance to ask questions, stay on track with your health goals, and ensure your care is well coordinated with other providers and specialists.

Each  patient is supported by a care team, including a registered nurse (RN), physicians, care coordinators, and care managers who work together to provide coordinated care focused on your unique health goals, needs, and overall well-being.

Program Requirements
• You have two or more long-term or chronic health conditions (our team will help see if you qualify)
• You give permission for Watts Healthcare to provide Chronic Care Management (CCM) services
• You agree that we can securely share your health information with other doctors or providers involved in your care, if needed
• You understand that only one healthcare provider can bill Medicare for CCM each month
• CCM services include regular phone check-ins with your Care Coordinator, who works closely with your Watts Healthcare provider
